First, in the GREEN area below, click on the Browse buttons to search
your hard drive for the file(s) you would like to upload.

Now, in the RED area below, give us the information we'll need
to know how to handle the file(s) you are sending and how to contact you.
*All information must be completed for the file(s) to be transmitted.

Then click the Upload File button below.

Now you will see a gray box indicating upload progress. 
This is not always working correctly at the present time. 
You should also see a green progress bar on the bottom of your browser.  This is currently more accurate. 

In any case, when your file(s) are done uploading correctly
you will see a new page saying "Thank you for your submission."
